Load on all claude.ai pages and only enhance Settings at runtime, making userscript-manager injection more reliable when starting from Billing. Add History API, popstate/hashchange, visibility, click, and short polling fallbacks for Claude SPA route changes.

Fix SPA navigation from Billing to Usage by loading on all Claude settings tabs. Replace the ambiguous "pp" label with "points".
